Health Services
The Health Services Office supports the District’s important work with students by providing services to students who become ill or require first aid during the school day.
The goal of the health office is to provide a safe healthy school environment for all students to be successful.
Don’t let your child be excluded. Be sure to have all mandated health exams and immunizations completed prior to attending school.
School Health Requirements
Parents and guardians are strongly encouraged to ensure their child’s health exams and immunizations are up to date. These include:
- Child health exam and updated immunizations: kindergarten, 6th grade, 9th grade, and any student new to District 131.
- Meningococcal Conjugate Vaccine: All 12th graders must show proof of receiving the vaccine on or after the 16th birthday.
- Dental: kindergarten, 2nd grade, 6th grade, and 9th grade.
- Vision: kindergarten, and any student enrolling for the first time in an Illinois school.
Any student without a child health exam or proof of updated immunizations is excluded from school as of October 15. For questions or more information, Click Here to read the Back to School Requirements letter or please contact your home school.
Required health records for Preschool through 12th grade
A child health examination is required for preschool, Kindergarten, 6th grade, 9th grade and those new to the district.
A dental exam is required for Kindergarten, 2nd grade, 6th grade, and 9th grade.
An eye exam is required for Kindergarten and those new to the district.
Immunizations are required for preschool, Kindergarten, 6th grade, 12th grade, and those new to the district.
These services are also available through the VNA Health Center at East Aurora High School and the VNA Mobile Health Clinic. To give permission for your child to receive care, please complete the digital consent form.

During the summer, forms can be dropped off to Kara Patrick at the Administrative Center, 310 Seminary Avenue, Aurora. In the Fall once school begins, forms can be turned into the VNA clinic directly or the Nurse’s office at East Aurora High School, 500 Tomcat Lane, Aurora. Physical and Immunizations
Students who have attended school in Illinois, or East Aurora School District 131 prior to the start of the current school year, are required to have all mandated exams and immunizations completed prior to attending school. Students that have transferred from out of state or out of country are required to submit a valid Illinois physical prior to attending school. Proof of immunizations must be submitted within 30 days after starting school.
The physical exam must be completed within one calendar year prior to the start of school and must fulfill all Illinois State Board of Education and Illinois Department of Public Health requirements and be dated within one year prior to first day of school. Physical exams will only qualify for sports physicals if ‘interscholastic sports’ is checked on the physical. The sports physical is valid for one year from date to date. All immunizations are required to be up to date regardless of grade of student.
Dental forms
Illinois State law now mandates that all kindergarten, 2nd, and 6th grade students present proof of a dental examination prior to May 15 of the school year. Dental examinations are now required for 9th graders. A waiver has been established by the Illinois Department of Public Health for students who show undue burden or lack of access to a dentist.
Vision examination forms
Illinois State law now mandates that all incoming kindergarten and students new to Illinois schools must present an eye exam report conducted by a physician who provides a complete eye exam or a licensed optometrist within one year prior to the required date. A waiver has been established by the Illinois Department of Public Health for students’ who show undue burden or lack of access to an ophthalmologist or optometrist.
Medications at school
All medication administration (including over the counter and prescription) during school hours requires a general Medication Authorization Form signed and dated by the student’s health care provider and the parent/guardian. No medications will be given during the school day if they can be administered outside of school hours. The primary responsibility for administering medication to a student rests with that student’s guardian(s).
Guardian(s) may authorize their child to self-administer a medication according to District and School code. The school health office must be notified and in agreement. This is meant for asthma, emergency and life-threatening medication only.
