Welcome to District 131!

How to Register a New Student

Students who are new to District 131 may begin their online registration, upload all additional documentation to the application, or email them to enroll@d131.org. (If you already have other students in the district, add the new student through the ParentVue Application. Do not start your application here.)

Additional Documentation: Upload or email photos or pdf files of the following documents to enroll@d131.org.
- - Birth Certificate: Original or certified copy
  - Parent/guardian’s driver’s license or photo ID
  - Affidavit of guardianship: If you are not the student’s parent or legal guardian and have assumed responsibility for a student for reasons other than access to the educational programs of the school district.
  - Proof of Address: You must provide three (3) documents in parent/guardian’s name. See examples listed below:
    - Title evidence, mortgage papers, or lease agreement
    - Utility bill for current months: (i.e. Nicor Gas, Com Ed, Comcast, city water)
    - Medical card with current address
    - Green Card/Matriculate Card with current address
    - Affidavit of residency (if applicable)
  - Previous school transfer form
  - Home Language Survey

Illinois State Board of Education Transfer Form: This document must be provided by the previous school if transferring from a public school within the State of Illinois.
Unofficial transcript (high school only): This document must be provided by the previous school.
State of Illinois Health Physical/Immunizations Exam, Dental and Vision Exams
Health Physical/Immunizations Exam: All preschool, kindergarten, 6th, 9th, grade students.
Dental Exam: All kindergarten, 2nd and 6th grade students.
Vision/Eye Exam: All kindergarten students.
Current IEP or any other special education records: If student was receiving services at previous school (if applicable)

School fees will be collected after the first day of school.

Los estudiantes nuevos al Distrito 131 pueden empezar su inscripción en-línea y adjunte los documentos adicionales a la aplicación o envié los documentos al correo: enroll@d131.org. (Si ya tiene a otros estudiantes en el distrito, agregue al estudiante nuevo mediante la solicitud en-línea en el portal para padres. No inicie su solicitud aquí).

- - Acta de nacimiento: original o copia certificada
  - Licencia de conducir/identificación con fotografía del padre/tutor
  - Declaración jurada de custodia legal: Si usted no es el padre o tutor legal del estudiante y ha asumido la responsabilidad de un estudiante, por razones distintas al acceso a los programas educativos del distrito escolar.
  - Comprobante de domicilio: Debe presentar tres (3) documentos con el nombre del padre/tutor. Consulte los siguientes ejemplos:
    - Título de propiedad, escritura de hipoteca o contrato de arrendamiento
    - Factura de servicio público de meses actuales: (por ejemplo: NicorGas, ComEd, Comcast, servicio de agua de la ciudad)
    - Tarjeta médica con domicilio actual
    - Visa de residencia permanente, matrícula consular con domicilio actual
    - Declaración jurada de residencia (si corresponde)
  - Documento de transferencia de la escuela anterior
  - Complete la encuesta sobre el idioma que se habla en casa

Las cuotas escolares se cobrarán después del primer día de clases.
